We are actively seeking a highly motivated, innovative and experienced leader to join our team as the Community Outreach Vehicle Driver. Compensation will be commensurate with experience.

Position Summary:

This position supports the Director of Outreach Programs with organizing, planning, and a strong focus on monitoring data and documentation of Retention in Care Unit (RICU) programs. The programs vary in focus from housing, patient navigation, peer work to connecting members to care. The ultimate goal of RICU programs are to ensure member who are lost to care, or who are at risk to become lost to care, are provided with the necessary bio psychosocial services to assure optimal health outcomes; and to ensure member who are looking to seek education or become certified as peers can be linked to appropriate programs. This position will provide administrative and clinical support to staff and report on programs to health services leadership to ensure Amida Care members receive the most appropriate level of care. 

Responsibilities:

  • Develop new policies and job descriptions for new grants within the RICU department.
  • Provide direction and oversight to Social Determinants of Health Programs
  • Develop work flows, monitor, analyze and report on departmental quality measures.
  • Develop protocol for monitoring notes inputted into Team Connect
  • Conduct chart reviews on monthly basis
  • Track success rate of the programs and develop and implement action plans to ensure future success.
  • Support and guide outreach staff for bi- monthly outreach staff meetings.
  • Collaborate and attend meeting with other Amida Care Departments to ensure collaboration, follow up of member referrals are being met.
  • Hire and retain quality staff and the on-going planning, training, coaching, evaluation, and appraisal of staff performance and development to ensure successful achievement of goals and objectives.

EDUCATION REQUIRED

  • Bachelor’s degree in science, healthcare administration or similar, or an equivalent combination of education and directly related experience in health care, specialized populations, diversity initiatives and HIV treatment and prevention.

EXPERIENCES AND/OR SKILLS REQUIRED

  • Three (3) years’ experience working with those living with HIV in a social service or medical setting; with a strong background in staff supervision experience.
  • Demonstrated knowledge of HIV case management.
  • Strong knowledge of Microsoft Office (Word, Vision, Excel).
  • Demonstrated judgment of safety, boundaries and confidentiality issues.
  • Demonstrate understanding and sensitivity to multi-cultural values, beliefs, and attitudes of both internal and external contacts.
  • Demonstrate appropriate behaviors in accordance with the organization’s vision, mission, and values.
