        Basic Function:<br />\r\n
        •&nbsp; &nbsp; &nbsp; &nbsp;Provide intake and assessment case management services to individuals<br />\r\n
        currently incarcerated.<br />\r\n
        •&nbsp; &nbsp; &nbsp; &nbsp;Assess, address and provide consultation to other staff regarding prison<br />\r\n

        Defenders Association, court officers, probation and parole offices.<br />\r\n

        Specific Responsibilities:<br />\r\n
        •&nbsp; &nbsp; &nbsp; &nbsp;Accept referrals from various sources for services to incarcerated<br />\r\n
        individuals<br />\r\n
        •&nbsp; &nbsp; &nbsp; &nbsp;Perform face-to-face needs assessment.<br />\r\n
        •&nbsp; &nbsp; &nbsp; &nbsp;Complete a service care plan with the client<br />\r\n
        •&nbsp; &nbsp; &nbsp; &nbsp;Maintain accurate and timely activity logs.<br />\r\n
        •&nbsp; &nbsp; &nbsp; &nbsp;Obtain updated medical and demographic data and informed consent of each<br />\r\n
        client.<br />\r\n
        •&nbsp; &nbsp; &nbsp; &nbsp;Maintain a reduced caseload and provide case management and advocacy to<br />\r\n
        the HIV+ individuals who are incarcerated or have recently been released from<br />\r\n
        prison.&nbsp; This includes being available to visit the client at the prison,<br />\r\n
        home, in the hospital and office visits.<br />\r\n
        •&nbsp; &nbsp; &nbsp; &nbsp;Assist clients in negotiating, arranging and linkage to medical care,<br />\r\n
        social services and other provider systems.&nbsp; Provide the client with<br />\r\n
        information and referral,&nbsp; &nbsp;arrange necessary services.&nbsp; Follow up and<br />\r\n
        monitor the support system on a monthly basis.&nbsp; Reassess client need as<br />\r\n
        appropriate.<br />\r\n
        •&nbsp; &nbsp; &nbsp; &nbsp;Work with care providers in the areas of hospital discharge planning and<br />\r\n
        home care needs.<br />\r\n
        •&nbsp; &nbsp; &nbsp; &nbsp;Provide specialized case management and advocacy by carrying a caseload<br />\r\n
        of individuals whose primary presenting problem is being symptomatic of HIV<br />\r\n
        disease and reentry to society following incarceration.<br />\r\n
        •&nbsp; &nbsp; &nbsp; &nbsp;Evaluate the need and assist clients to be admitted to substance abuse<br />\r\n
        and/or mental health treatment.<br />\r\n
        •&nbsp; &nbsp; &nbsp; &nbsp;Consult with other members of Action Wellness staff including direct<br />\r\n
        service staff, housing specialist, prevention case managers, volunteers and<br />\r\n
        others as needed.<br />\r\n
        •&nbsp; &nbsp; &nbsp; &nbsp;Identify resources and communicate with agencies and organizations<br />\r\n
        providing services to incarcerated and recently incarcerated individuals.<br />\r\n
        •&nbsp; &nbsp; &nbsp; &nbsp;Attend weekly one on one supervisory meeting.<br />\r\n
        •&nbsp; &nbsp; &nbsp; &nbsp;Participate in Action Wellness staff meetings and in-service trainings.<br />\r\n
