We need passionate agents of social change,<br />\r\n vanguards of public health, and practitioners of racial justice to join our<br />\r\n fight.<br />\r\n <br />\r\n POSITION OVERVIEW:<br />\r\n <br />\r\n Rafiki Coalition and San Francisco AIDS Foundation have partnered to bring<br />\r\n experienced partners, local resources and providers, and community members to<br />\r\n inform and generate innovative, creative, and holistic approaches to address<br />\r\n sexual health disparities and related health and social issues within San<br />\r\n Francisco’s Black and African American communities. The Capacity Building<br />\r\n Manager, Black Health uses a Black Queer wellness framework to establish a<br />\r\n multi-year strategy for increasing access points to sexual health services<br />\r\n for Black and African community members. The Coordinator will ensure that<br />\r\n efforts are community driven, holistic, accessible, welcoming, sex-positive,<br />\r\n trauma informed and culturally resonant with the goal of establishing a<br />\r\n comprehensive range of linked, integrated sexual health services for and by<br />\r\n Black and African American communities.<br />\r\n <br />\r\n The Manager will work extensively with community partners to adapt successful<br />\r\n models and create one-stop shop models, within African American culturally<br />\r\n resonant frameworks, providing holistically integrated HIV, STD, and HIV<br />\r\n testing; linkage and navigation to pre-exposure prophylaxis (PrEP) and<br />\r\n post-exposure prophylaxis (PEP), HIV care, HCV and STD treatment, primary<br />\r\n care, safer sex supply distribution, education and behavioral health<br />\r\n services.<br />\r\n <br />\r\n The Manager will work extensively with SFAF and Rafiki leadership to create<br />\r\n safe and thriving new community gathering spaces that apply a<br />\r\n multi-generational approach to supporting and promoting individual, family,<br />\r\n and community wellness by harnessing the inherent creativity, strength,<br />\r\n pride, and fellowship that exists in San Francisco’s Black and African<br />\r\n American communities.<br />\r\n <br />\r\n ESSENTIAL DUTIES AND RESPONSIBILITIES:<br />\r\n <br />\r\n Program Development: Responsible for supporting Rafiki’s efforts to<br />\r\n establish sexual health services for and by Black and African American<br />\r\n communites. Includes adapting effective service models, proposing<br />\r\n implementation options, and establishing protocols and handbooks. Make<br />\r\n appropriate adjustments to meet contract goals as set by SFDPH, SFAF, Rafiki,<br />\r\n and collaborating partners.<br />\r\n <br />\r\n Project Management: Provides project management support to SFAF, Rafiki<br />\r\n Coalition, and collaborating partners to ensure that work plan deliverables<br />\r\n and milestones are achieved. Assists with meeting facilitation,<br />\r\n documentation, and project-related research. Manages consultant<br />\r\n relationships.<br />\r\n <br />\r\n Collaboration: Maintains and supports partnership between SFAF, Rafiki<br />\r\n Coalition, and emerging partners. Partners with Clinical Assistants Manager<br />\r\n and ensures the community health worker pipeline is integrated into the<br />\r\n project model.<br />\r\n <br />\r\n Evaluation: Collaborate with the Senior Director of Community Partnerships &<br />\r\n Engagement to establish an evaluation strategy. Develop and implement program<br />\r\n specific process and outcome evaluations.<br />\r\n <br />\r\n Collegial Teamwork: Develop and maintain relationships with current and<br />\r\n potential partners; focusing on regular collaborations and interagency<br />\r\n services dovetailing.
