This position is responsible providing health promotion and education to individuals who need additional support with adhering to their prescribed regimen and health care plan. Treatment Adherence Coordinators will develop and maintain external and internal relationships with services providers in order to provide direct treatment adherence services to Amida Care members.

ESSENTIAL FUNCTIONS
Essential functions are job duties and responsibilities that must be performed to accomplish the purpose/ goals of the job.
•       Follow policies, procedures and protocols to ensure member’s adherence to treatment and care plan.
•       Assess adherence of member’s using assessments on quarterly basis.
•       Develop personalized treatment plan and educational tools that will assist and improve treatment and health of member.
•       Develop and maintain relationships with Primary Care Physician (PCP) site requesting treatment adherence services in order to integrate into the care team.
•       Conduct home visits or field visits, and develop a schedule with members to meet their needs (ie. DOT, weekly, monthly).
•       Provide treatment adherence services to Amida Care members; including but not limited to DOT, appointment escorts, treatment adherence education, and treatment adherence tools.
•       Ensure members attend regular PCP appointments and monitor progress through escorts, blood work results and case conferencing with PCP.
•       Case conferences with Amida Care staff involved with member and provide updates, as well as seek assistance as needed to compliment the care of member.
•       Refer members to case management and other social services as necessary and follow up on all referrals.

MINIMUM JOB REQUIREMENTS

•       Bachelor’s degree in Human Services preferred or significant relevant work experience in HIV treatment adherence/care coordination.
•       Three years’ experience working with those living with HIV in a social service or medical setting preferred.
•       Demonstrated knowledge of HIV treatment adherence.
•       Demonstrated judgment of safety and confidentiality issues.
•       Demonstrated intermediate knowledge of Microsoft Office and CRM databases.
•       Demonstrated excellent communication skills: oral and written. Ability to maintain routine records pertaining to participant utilization of services as well as to prepare summaries.
•       Demonstrated knowledge of sociological and behavioral factors influencing the behavior and attitudes
of priority group members’ relative to program to which assigned.
•       Demonstrated judgment of safety, boundaries and confidentiality issues.

•       PHYSICAL DEMANDS: The physical demands described here are representative of those that must be met by an employee to successfully perform the essential functions of this job. While performing the duties of this job, the employee is regularly required to communicate. The employee occasionally is required to sit at a desk and sometimes use a computer. The employee is constantly required to stand; walk; ride the bus/subway to all 5 boroughs of New York City.
The employee commutes, approximately 75% of the time, to meetings, trainings, and home visits as scheduled throughout the year. The employee spends approximately 25% of the time writing and/or keyboarding.

employee encounters while performing the essential functions of this job.
While performing the duties of this job, the employee works mostly outside and sometimes in a variety of weather conditions. The noise level in the work environment is usually moderate and sometimes noisy.
