We are looking for someone who can provide comprehensive support to LGBTQIA+ patients who are receiving services at Philadelphia FIGHT. Provide outpatient social work services to LGBTQIA+ patients, including psychosocial assessments, counseling, patient education, and resource linkage to outside services as needed, including surgical centers. The social worker acts as an advocate for LGBQTIA+ patients while connecting them to necessary services within and outside of FIGHT including medical, mental health, and social service agencies. Assist in organizing and facilitating LGBTQIA+ trainings for all staff. Connect with other pride navigators in the region to stay up to date on resources and build partnerships. Create and maintain gender affirming resource guide and associated handouts.

Philadelphia FIGHT Pediatrics and Youth Health Empowerment Project Health Center (YHEP) together provide comprehensive and culturally effective health care to children and young adults from birth to 24 years old, with a particular focus on immigrants and LGBTQ+ children and youth.

The patients and staff at Philadelphia FIGHT are diverse, representing a broad spectrum of race, religion, gender identity, sexual orientation, HIV status, and physical ability. As such it is imperative that candidates demonstrate sensitivity to the needs of various populations.

Job Responsibilities

• Serve as first point of contact for LGBQTIA+ patients and their families.

• Provide outpatient social work services for LGBTQIA+ patients in collaboration with providers and support staff across FIGHT.

• Collaborate with Assistant Medical Director of YHEP Health Center to identify unmet needs, gaps in care, and strategic planning and development for this population

• Assist with referring and assisting patients in scheduling outpatient specialty appointments and being familiar with public and private insurance options.

• Establishing communication with clinical team to enhance supportive patient care, stay up to date on services and patient navigation process, and to develop referral process and workflow that optimizes patient care, access, and clinical experience.

• Assisting with technological aspects including development and presentation of LGBTQIA+ focused training, delivering them in person and virtually.

• Answering telephones, EMR messages, and emails, responds to questions and/or obtains information for or about the FIGHT's programs as appropriate. Forwards calls to appropriate clinical staff or Manager if necessary.

• Build and maintain website and other social media platforms around gender-affirming care services, resources, and educational materials.

• Maintaining up-to-date resource guide for LGBTQIA+ resources in the region

• Support FIGHT staff in outreach activities including attending community events.

• Collaborate with the PrEP team to advocate for increased PrEP access for LGBTQIA+ population and assist with patient-driven sexual health resources.

• Advocate for policies and system improvements to make FIGHT more LGBTQIA+ inclusive and affirming.

• Network with and stay informed about other LGBTQIA+ service providers to be able to provide warm handoffs and seamless referrals to community resources.

• Adheres to departmental and institutional policies and procedures while actively supporting the goals of FIGHT's LGBTQIA+ program .

Qualifications

• Master's in Social Work (MSW) or Bachelor's in Social Work (BSW)

• Knowledge and ability to ask appropriate clinical questions and elicit patient information to determine appropriate action(s) for LGBTQIA+ patients.

• Demonstrate significant knowledge of local resources for LGBTQIA+ patients.

• Extensive knowledge working with the LGBTQIA+ community in general and, particularly, trans and non-binary individuals. Lived experience preferred.

• Understanding of the unique healthcare needs of the LGBTQIA+ community, specifically for trans community

• Interest in supporting LGBTQIA+ program development within the community health model of low-barrier health care.

• Minimum 1 year working in a medical setting.

• Experience and competency with word processing, excel, and EMRs preferred with data entry skills a plus.

• Excellent verbal communication and telephone decorum.

• Ability to work within a team to develop and maintain high quality health care
