We are actively seeking a highly motivated, innovative and experienced leader to join our team as the Community Health Outreach Worker – Transgender Nonbinary (TGNB). Compensation will be commensurate with experience.
Position Summary:
The Community Health Outreach Worker (CHOW) for the Transgender Nonbinary (TGNB) Program is a part-time position that assists with the outreach, education and counseling of a specified group of members; consistent with the philosophy, goals and objectives of Amida Care. The CHOW assists clients in adhering to their individual pre- and post-surgical care management plans, provides lived-experience counseling, and offers additional support to those with the greatest needs, particularly young TGNB people. The incumbent may also be an Amida Care Plan member.
Responsibilities:
- Provide assigned groups of members with general information pertaining to the Plan and healthcare access.
- Facilitate access to surgeries and related pre- and post-care medical visits.
- Provide home visits for specific members who may benefit from added support.
- Conduct pre- and post- surgery assessments and provide education on expectations and achievable goals.
- Foster trust in healthcare systems and promote healthy habits in young TGNB members.
- Act as patient advocate in specific situations and/or discussions with care providers.
- Counsel designated clients on basic harm reduction information and available resources.
- Provide social support and enhancement of self-esteem to assigned clients, with a focus on the young population.
- Assist Coordinator as needed.
- Identify barriers to care.
- Promote Sexual Behavioral Assessment and app uptake at sponsored sites and relevant events.
- Support data collection and entry.
- Perform outreach calls.
- Perform other duties as assigned.

EDUCATION REQUIRED
- High School Diploma or equivalency.
EXPERIENCES AND/OR SKILLS REQUIRED
- Demonstrated experience/ participation in community activities.
- Demonstrated judgment of safety, boundaries and confidentiality issues.
- Demonstrated competency in using Microsoft Outlook, Word, Excel, and ability to learn to use proprietary applications.
- Demonstrate understanding and sensitivity to multi-cultural values, beliefs, and attitudes of both internal and external contacts.
- Demonstrate appropriate behaviors in accordance with the organization’s vision, mission, and values.
