HCV Care Patient Navigator
The HCV Care Patient Coordinator will provide clinic-based, off-site and
other community level support to HIV/HCV positive clients.

Description:
Greet and acknowledge all patients according to agency standards
Receive, record and relay messages accurately, completely and legibly
Speak with others using clear, professional and polite language
Develop and maintain positive working relationships with others, support the
ACG team to reach common goal
Serve as a team member of the Hepatitis C outreach, education, and treatment
division of AIDS Care Group
Represent ACG at community health fairs
Share driving responsibilities of the Medical Treatment Unit
Communicate with patients, employees, and other individuals to answer
questions, disseminate or explain information, take orders, and address
complaints
Intake and assessment of new HIV/HCV positive patients
Meet, counsel and screen/test new HIV/HCV potential positive clients at
Community correctional centers; Halfway Houses, Health Fairs, Recovery
centers/homes, Social Events, Treatment clinics, at any of our three agency
sites and in our Mobile Treatment Unit
Review benefit/insurance status and enable this coverage to better facilitate
AIDS Care Group treatment and to better facilitate their pharmaceutical
coverage
Obtain referrals for patients who are obtaining MONO HCV care at the AIDS
Care Group
Link these clients to all provisions of care
Act as the main communicator and coordinator for assigned drug and treatment
centers as well as other sites in the collar counties (Berks, Bucks, Chester,
Delaware and Montgomery) around Philadelphia. This to be done under close
association with the Division Director
Monitor and help navigate all Co-infected and MONO HCV patients through the
process of completing HCV treatment; this includes seeing the HCV client
through all months of treatment, between prescription blood work appts, and
the Sustained Viral Response (SVR) appointments of both 6 & 12 weeks
Maintain and update HCV & HIV testing data bases (outside of current STD
related testing data)
Build all physical HCV & HIV testing folders as needed
Assist the HCV Supervisor in the management and movement of HCV medications,
ensuring that all refills are completed, and all medications are obtained
when needed from several different pharmacies including Elwyn, Silver
Scripts, and Briova
Serve as back-up for the HCV Supervisor in compiling and completing all HCV
applications, and present to the respective providers
Review HCV files and provide support to medical staff in completing paperwork
for these files
Work evening hours as assigned and occasional weekend hours to coordinate
with agency efforts
Demonstrate the ability to travel to various satellite locations and to best
address these client needs
Learn HIV/HCV testing methods and provide these rapid and other tests on
clients and in the agency clinic locations
Regularly attend meetings, trainings and especially correctional trainings,
meetings and events to promote the goals of this special treatment
initiative. Attend conferences and report findings to staff per trainings at
agency events
Maintain company vehicle for transportation of clients as directed by
Facilities Director
Other duties as assigned
Requirements:
High School Diploma (or GED equivalent)
Experience working in the medical field required
Experience working in mental health preferred
Weekend and evening availability is required
A working knowledge of the community and the ability to link clients to
services in the community when necessary
Experience with Electronic Medical Records systems
Valid Driver’s License
Must be able to comfortably work with people of all lifestyles and from
various socioeconomic backgrounds

Frequently required to stand, walk and sit for longer periods of time
