Job Description
Purpose
This position provides direct client services for participation in the
Delaware Housing Assistance Program (DHAP) and helps transition people from
DHAP to other stable, long term housing
Essential Duties
1. Transition Planning (at least 50% of time and effort)
• Explore available housing resources through outreach and engagement with
Delaware’s affordable housing providers, including public housing
authorities and subsidized apartment complexes, private landlords and
property managers
• Create and continuously update a list of available housing options to
ensure housing choice in the transition planning process
• Meeting with DHAP program participants to create customized housing
transition plans that would ensure a seamless exit from the DHAP program to
other suitable housing
• Guide program participants through the application process for
alternative housing options
• Contact clients regularly to track progress in their transition plans
• Communicate regularly with case managers, landlords, property managers,
and others who provide services that promote housing stability
• Report on transition outcomes
2. DHAP Responsibilities
• Activate clients from the waiting list into the program, including:
reviewing program eligibility; conducting interviews; providing housing
information services; calculating rent subsidies; reviewing leases;
communicating with clients, landlords, case managers, utility providers, and
other service providers; completing housing placement process
• Integrate vocational assessment and employment assistance for program
beneficiaries into program operations to encourage client wellness,
stability, and self-sufficiency.
• Recertify clients annually
• The Housing CM is responsible for 50% of all clients
• The Housing CM works with clients residing in all counties
• The Housing CM works primarily with clients over the age of 50
• Disburse information about housing options to persons seeking information
about resources in Delaware
• Update and maintain client case files and housing program database on an
ongoing basis.
3. Other responsibilities:
• Professional development activities including: participation in webinars,
workshops, and conferences to enhance program quality and comply with federal
regulations
• Other duties as assigned
Requirements
A minimum of a Bachelor degree with a minimum of 2 years’ experience
providing direct social services required. Knowledge of and experience of
housing assistance programs and a background in case management and HIV
preferred.
Special Conditions
DHAP is a statewide program, and as such, requires statewide travel. The
person holding this position must hold a valid driver’s license and have a
reliable vehicle. Mileage reimbursement is offered for appropriate work
related travel.
Salary range
$38,000 to $40,000
