We are actively seeking a highly motivated, innovative and experienced leader to join our team as the Supervisor of Peer Programs. Compensation will be commensurate with experience.

Position Summary:

The Supervisor of Peer Programs is responsible for oversight and day to day supervision of Community Health Outreach Workers (CHOWs). The Supervisor of Peer Programs works closely with other departments and programs such as Member Services (NMO process), Pharmacy department, Integrated Care Teams and External Care Coordination Specialist to support Amida Care members to remain in care, resolve bio psychosocial issues, and achieve optimal health outcomes. The Supervisor of Peer Programs will collaborate and partner with outside agencies to help expand peer worker programs at Amida Care, such as Workforce Initiative Network and Consumer Workforce Innovator Project. The Supervisor of Peer Programs will also provide support and guidance to peers who are interested to become NYS Peer certified. 

Responsibilities:

  • Manage day-to-day activities as well as the community relationships involved in the Community Health Outreach Workers.
  • Responsible for the hiring and retention of quality staff and the on-going planning, monitoring, training, coaching, evaluation, and appraisal of staff performance and development to ensure successful achievement of goals and objectives.
  • Provide programmatic and supervision to Community Health Outreach Workers. Provide supervision on-site and in the field at sites in the community as needed. Utilize audits results and documentation reviews to conduct individual data-driven supervision to ensure staff are meeting program objectives and to improve program delivery and overall effectiveness of the programs.
  • Provide support to CHOWs to ensure members are maintained in medical care through the coordination of care; Ensure collaboration with Amida Care’s Integrated Care Teams (ICTs); Ensure staff address needs in a timely manner and that successful outcomes are achieved. Develop and maintain a close connection with Care Coordination staff at PCP sites; collaborate with Care Coordination staff to facilitate the smooth transfer of information and services.
  • Collaborate with agencies to expand the number of agencies who participate in Consumer Workforce Innovator Project (CWIP) and implementation of new initiatives. Provide support and oversight to peers in the CWIP who are interning at partnering agencies, to ensure quality and adherence to policies and procedures.
  • Expand peer certification with CHOWS. Support those participants of WIN and participants of CWIP interested in becoming NYS peer certified, by assisting in finding practicum experience.

EDUCATION REQUIRED

  • Bachelor’s degree in Human Services or equivalent job experience in health services, community based organizations and/or community outreach required.

EXPERIENCES AND/OR SKILLS REQUIRED

  • Two (2) years Supervisory experience preferred, or two (2) years’ experience training peers or being a peer preferred.
  • Knowledge of NYS Peer Certification process
  • Five (5) years’ experience working with individuals living with HIV/AIDS or other severe chronic conditions with complex needs is required. Experience working with Community Based Organizations and/or Health Centers is strongly preferred.
  • Ensure member confidentiality and adhere to Confidentiality and Health Insurance Portability and Accountability Act (HIPPA) policies and regulations policies and regulations.
  • Strong knowledge of Microsoft Office (Word and Excel).
  • Demonstrate understanding and sensitivity to multi-cultural values, beliefs, and attitudes of both internal and external contacts. Demonstrate appropriate behaviors in accordance with the organization’s vision, mission, and values.
