With<br />\r\n compassion at its core, our approach uses best practices and evidence-based<br />\r\n programming to pave the<br />\r\n way for personal and community transformation.<br />\r\n Way Home is a new Rapid Re-Housing program operated by SELF, Inc. offering<br />\r\n rental assistance to<br />\r\n homeless individuals with specialized services for the LGBTQ+ community. This<br />\r\n program provides one<br />\r\n year of subsidized rent to program participants to support them in<br />\r\n transitioning from homelessness<br />\r\n to renting their own units. The program was designed to meet the huge need<br />\r\n for housing in the<br />\r\n LGBTQ+ community, and particularly in queer and Tran’s communities of<br />\r\n color, who experience<br />\r\n disproportionate rates of homelessness and have barriers to accessing safe<br />\r\n shelter. All<br />\r\n individuals, regardless of gender or sexual orientation are welcomed to<br />\r\n apply.<br />\r\n Position Summary: The Way Home Housing Support Coach will work with single<br />\r\n adults, couples, and/or<br />\r\n families as they transition from homelessness into privately rented units<br />\r\n with subsidized rental<br />\r\n assistance through the Rapid Re- Housing model. The Housing Support Coach<br />\r\n will provide coaching and<br />\r\n supportive services such as connecting participants with benefits and<br />\r\n resources, teaching life and<br />\r\n social skills to maintain long-term housing stability, assisting participants<br />\r\n with educational,<br />\r\n employment skills building opportunities, financial literacy/budgeting and<br />\r\n daily living skills, and<br />\r\n providing referrals to treatment for mental health, substance use, or medical<br />\r\n care when requested.<br />\r\n The Housing Support Coach will also be a liaison with private landlords, and<br />\r\n will support with<br />\r\n conflict resolution and tenant advocacy as needed. The Housing Support Coach<br />\r\n will play a vital role<br />\r\n in engaging participants through a person centered, trauma informed and harm<br />\r\n reduction approach.<br />\r\n This position is a full-time salaried, exempt position.<br />\r\n Key Duties and Responsibilities<br />\r\n • Manage caseload of up to 25 households who recently moved out of<br />\r\n homelessness into private<br />\r\n units.<br />\r\n • Complete intakes with new referrals. Support households in gathering<br />\r\n all necessary<br />\r\n documentation for program enrollment and complete individualized goal-based<br />\r\n service plans.<br />\r\n • Maintain consistent contact with households throughout the entire year<br />\r\n of rental assistance.<br />\r\n Evaluate the level of support needed for each household.<br />\r\n • Connect participants to trans-competent supportive services for mental<br />\r\n health,<br />\r\n gender-affirming care, legal support, or other referrals as needed.<br />\r\n • Meet participants where they are, and provide regular home visits at<br />\r\n their private units in<br />\r\n neighborhoods all throughout the city of Philadelphia.<br />\r\n • Work with participants on their own goals for employment and<br />\r\n education. Provide referrals for<br />\r\n workforce development and job training programs to support participants in<br />\r\n increasing income.<br />\r\n • Support participants in creating plans for permanent housing beyond<br />\r\n the subsidy, and to help<br />\r\n them transition out of the program.
