Compensation will<br />\r\n be commensurate with experience.<br />\r\n <br />\r\n Position Summary:<br />\r\n <br />\r\n This position will be a part of a Health Navigation for Lost to Care, HIV<br />\r\n Unsuppressed grant. The Community Liaison will be an individual who will use<br />\r\n their lived experience to help engage members not in effective care. The<br />\r\n Community Liaison will be responsible for outreaching and connecting members<br />\r\n lost to care, with high emergency department (ED) and inpatient (IP)<br />\r\n utilization and whose HIV is unsuppressed. The Community Liaison will<br />\r\n outreach members who are in the ED or IP to attempt to reengage in care and<br />\r\n provide connection to services. This position will function as a liaison<br />\r\n between community providers and staff at assigned facilities and the<br />\r\n Integrated Care Teams (ICTs) at Amida Care.
